Request an Employer Contribution Kit
To arrange for employer contributions to be made into your Telstra Super Personal Plus account, you will need to provide your employer with an Employer Contribution Kit.
The Employer Contribution Kit contains:
- Telstra Super Letter of Compliance
- Telstra Super Personal Plus Product Disclosure Statement
- Telstra Super Employer Contribution Remittance Advice.

Over the phone - call our Member Services Consultants on 1300 033 166 from 8:00am
to 5:30pm (Melbourne time) Monday to Friday.
What happens next?
Once you have received your Employer Contribution kit, provide it to your employer, with your completed Standard Choice Form (which your employer will provide to you). Your employer will then work directly with Telstra Super to arrange for your Superannuation Guarantee contributions to be made into your Telstra Super Personal Plus account.
