Before you open a TelstraSuper Personal Plus account, please read the TelstraSuper Personal Plus Product Disclosure Statement (PDS) and Target Market Determination (TMD).
Investment and Insurance
As part of your application, you will need to make a decision on how you'd like your super invested and if you'd like insurance cover. If you don't make an investment choice, you'll automatically become a MySuper member of TelstraSuper Personal Plus. More information about our investment options and the type of insurance cover we offer are available in our Additional Information About Your Super Guide, Investment Guide, Personal Plus Insurance Guide and on our website.

Before you consolidate your super, you should consider the implications it may have, including fees, taxes and insurance cover. You may wish to talk to a financial adviser.
Third-party representatives
Do not complete this form if you are a third-party representative or opening an account on behalf of someone else. As we require additional information, you will need to complete a TelstraSuper Personal Plus Application form.
Distributors
